Uloom al Quran by Mufti Taqi Uthmani of Karachi is a very good book for a beginner student of Quran which answers all of the above questions and many more.
Although the book packs a lot of research, the simple writing style makes it easy and light to read.
Originally written in Urdu, this book has been translated in English and can be found at any book store which stocks Pakistani Islamic books.
